When the requirement is enabled, Discord displays a modal before opening the channel. This is not a form builder: its fields are fixed and stored with the ticket for use in the channel and its logs.

Configure the initial modal

Module setup requires Manage Channels. The command selects the panel channel and accepts an optional ticket category.
  • Reason — Requirement: Required · Limit: 900 characters · Purpose: Describes the request and is associated with the ticket.
  • Evidence — Requirement: Optional · Limit: 700 characters · Purpose: Adds context or textual evidence; it is not a file upload.
Module setup enables require_reason. The Dashboard can turn that requirement off afterward. When disabled, opening does not request the initial modal; this does not create configurable forms.
Evidence is text. A requester can attach images or files in the private channel if their Discord channel permissions allow it.

Current limitations

Headless does not provide editable questions, reusable forms, multiple forms per option, or a .ticket forms subcommand. The panel message can explain what to include in a reason, but it cannot change modal fields.
1

Configure Tickets

Enable the module from the Dashboard or use the Syntax card to set the panel channel and require an initial reason.
2

Describe the request

Edit the panel message in the Dashboard to explain what the reason should contain.
3

Test both fields

Open a test ticket and verify that the required reason and optional evidence appear in its channel.
4

Test without the modal

If you turn off require_reason in the Dashboard, test again to confirm that the ticket opens without requesting either field.
